Nurse Staffing User Application

Are you new to submitting nurse staffing data? Completing the form below will get you started on fulfilling state-mandated data submission for your facility. A facility’s primary user for nurse data submission must have written authorization from their facility CEO or authorized designee. That authorization is needed as part of the application form. The primary user then can provide authorization for your facility’s secondary user. Contact us at customerservice@team-iha.org or 866-262-6222 with any questions.

Upload written authorization from your facility’s CEO or authorized designee. Secondary authorization can come from the primary user.
User Account and HIPAA Compliance: Users must maintain a current password to continue system access and/or system generated reports and other notifications. Passwords expire every 90 days. Email notifications are sent 10 days prior to an account’s deactivation. A primary user is designated for each facility in order to manage user accounts. Primary users must contact us immediately of a user’s status change (e.g. separation of employment, or duties and ensure inactive accounts are removed from our system.
